Please read and initial each statement below:

I understand and agree that I do not have legal right over foster animals that I foster for Kanoa Rescue Foundation. I understand and agree that I am not able to rehome, give away, sell, adopt out, transfer etc. my fosters through Kanoa Rescue Foundation.

I understand and agree that foster animals through Kanoa Rescue Foundation can only go to veterinarians that are approved by the organization. I understand and agree that I will NOT be reimbursed if I am to take the animal to an unapproved veterinarian. I understand that I will NOT be reimbursed if I make a payment/donation to a veterinarian approved by Kanoa Rescue Foundaiton.

I understand and agree that I will NOT be reimbursed for any items I buy for my foster. I understand and agree that reimbursement for items for my foster must be approved by a Kanoa Rescue Foundation board members BEFORE I buy it and that I must have my receipt for the item.

I understand and agree that I am not able to make medical decisions for my foster animal through Kanoa Rescue Foundation.

I understand and agree that I will keep up with medical care of my foster animals which includes but is not limited to, routine and follow up appointments, vaccinations, spay/neuter procedures, heartworm/flea/tick prevention, and treatment for illnesses. (The rescue is responsible for all financial expenses for said treatments).

I understand and agree that I am responsible for providing transportation for my foster animals through Kanoa Rescue Foundation to events, veterinarian appointments, etc. I understand and agree that if I am not able to do this that I am responsible for finding transportation accommodations for my foster.

I understand and agree that if I decide that I am no longer able to foster an animal that I will give a minimum of at least 2 weeks notice for the RESCUE to find new accommodations. I understand and agree that I am not able to place the foster animal in a new environment/with new people without the approval of a Kanoa Rescue Foundation board member. I understand and agree that I am NOT going to place this animal in an animal shelter.

I understand and agree that all items provided/paid for by Kanoa Rescue Foundation will be returned to the rescue after I no longer need them for my Kanoa Rescue Foundation foster animal.

I understand and agree that my foster will primarily be an indoor animal (excluding some livestock/farm animals).

I understand and agree that I will provide a loving and safe home for any foster animal I have through Kanoa Rescue Foundation. I understand and agree that I will provide all needed training and socialization for my foster animal.

I understand and agree that Kanoa Rescue Foundations reserves the right to pull any foster animal I have through them at any given time without providing any specifications.

I understand and agree that I am required to bring my foster animal to events after all their medical is complete and they have been approved to be in public places.

I understand and agree that I will keep Kanoa Rescue Foundation up to date on my foster animal(s) through them including but not limited to their health, behavior, temperament, etc. I understand and agree that I will be able to provide pictures and videos of my foster animals through Kanoa Rescue Foundation.

I understand and agree that if I need to board my foster animal it must be in a Kanoa Rescue Foundation approved facility. I understand and agree that I will not receive reimbursement for boarding if it has not been pre-approved by a Kanoa Rescue Foundation board member.

I acknowledge that everything that I have written, initialed, and signed in this document is TRUE and that none of the information that I have provided is FALSE.

I understand that this is a legally binding contract and agree to all its terms and conditions.

Electronically sign and date that you acknowledge all information you provided to be true and that you agree to this contract.

Kanoa Rescue Foundation reserves the right to deny/refuse any foster application for any reason without specification to the applicant.

Kanoa Rescue Foundation reserves the right to terminate any foster from volunteering or participating in the program at any time without specification.
